来那度胺的作用机制及其在罕见免疫介导炎症性疾病中的应用进展

Mechanisms of Action of Lenalidomide and Advances in Its Application in Rare Immune-Mediated Inflammatory Diseases

  • 摘要: 来那度胺作为第二代免疫调节酰亚胺类药物,在降低神经毒性的同时增强了免疫调节效应,通过cereblon(CRBN)依赖及非依赖机制协同作用于适应性与固有免疫反应,并表现出抗血管生成和抗纤维化等多重生物学效应,近年来已逐步被探索用于罕见免疫介导炎症性疾病(immune-mediated inflammatory diseases, IMIDs)的治疗。现有研究提示,来那度胺在免疫球蛋白G4相关性疾病和白塞病等疾病中具有潜在疗效,但相关证据主要来源于病例报告,其最佳剂量、适用人群及长期安全性仍需进一步验证。本文综述来那度胺的作用机制及其在罕见IMIDs中的应用研究进展,为未来机制探索和临床实践提供参考。

     

    Abstract: As a second-generation immunomodulatory imide drug, lenalidomide enhances immunomodulatory effects while reducing neurotoxicity. It synergistically regulates adaptive and innate immune responses through both cereblon (CRBN)-dependent and independent mechanisms, and also exerts multiple biological effects including anti-angiogenesis and anti-fibrosis. Therefore, it has been gradually explored for the treatment of rare immune-mediated inflammatory diseases (IMIDs) in recent years. Existing studies suggest that lenalidomide has potential therapeutic efficacy in diseases such as immunoglobulin G4-related disease and Behçet'sdisease. However, relevant evidence is mainly derived from case reports, and its optimal dosage, applicable populations and long-term safety remain to be further verified. This review summarizes the mechanisms of action of lenalidomide and the research progress of its application in rare IMIDs, with the aim of providing references for future mechanistic exploration and clinical practice.
